In fact, it comes from the section in the PERL code where information is printed to the output file. It says that, when defining equiliibrium, "...(2) continuation for a specified fraction of the total run without an increase in USS (perfect and singly mismatched). This last part is the problem, it does not consider the lack of an increase in one-offs. It does print out the number of one-offs and the mean for the current printing interval, but does not consider them when defining equilibrium.
